What is your phone number?
Our main phone number will direct you to all the areas you want to contact in the building except for the pharmacy. Our main phone number is 503-362-9334.

How do I contact the PBG Pharmacy?
For your convenience, we have a full service pharmacy right here in our building. The pharmacy hours are 8:30 AM-5:30 PM Monday through Thursday and 8:30 to 5 PM Friday and the number is 503-362-4874.

How do I become a patient at PBG?
If you have not yet been accepted by one of our providers as new patient, please print/download and fill out our New Patient Request Form and either drop it off or mail it into Physicians Building Group. If you prefer to do it by phone, you can call our main number (503-362-9334) and leave your information

How do I make an appointment?
Please call our main number 503-362-9334, and follow the prompts given by the automated system. You will be directed to a staff member who can schedule an appointment with your provider.

If you have signed up, you may also make appointments via the patient portal.
What are your office hours?
Our office is open from 8:30 AM to 5 PM Monday through Friday. Our phones hours are 8:30 AM to 12:00 PM and 1:30 to 4:30 PM, weekdays only.
What should I do to be prepared for my office visit?
Parking fills up from time to time through the day, so please allow extra time for this. Please try and be in the building at least 15 minutes prior to your appointment time to allow time for checking in.
Please bring in your insurance card and medication list to every visit. Remember that co-pays are due when you check in prior to your visit.
If there have been any significant changes to your health, such as emergency room visits or hospitalizations since your last visit, please let our schedulers know ahead of time so that pertinent records can be obtained and there will be adequate time in your appointment to go over these changes.
What if it’s my first office visit?
If this is your first office visit, we will need your Patient Registration Form and your Medical History Form completed prior to your visit. We also would like to have your prior medical records and the Medical Records Request Form will be needed to go to your previous provider for this. It is best if you can do this ahead of time, but if not, we can assist you with this at your first office visit. We will also need you to review our Notice of Health Information Privacy Practices and you can either print out a Privacy Statement Form to sign and bring in at or prior to your visit.

What are the lab hours and policies?
Our full service in house laboratory is open from 8:30 AM to 4:30 PM Monday, and 7:30 AM to 4:30 PM Tuesday through Friday. We prefer to schedule labs, but walk-ins and same day orders can be worked in as well. We are happy to take orders from consultants outside of our facility.

Although we run most labs on site ourselves, we also collect specimens for tests that we do not perform and send them to the appropriate outside facility. You may receive bills from more than one place, depending on where the labs were run.
What if I need assistance after hours?
If it is a life threatening emergency, call 911 for assistance. If not, please call 503-362-9334, which is the same telephone number you would call during the day for your provider. An answering service will assist you in reaching the on-call provider. Please have pharmacy numbers or other pertinent information ready for your call back from the on-call provider.
How do I obtain a prescription refill?
Please contact your pharmacist for prescription refills at least three full business days before your supply runs out. They will contact your provider for refills.

If you have signed up, you may also request prescription refills via the patient portal. You should still make your request three business days ahead of time.
How do I find out about my test results?
Please discuss with your provider how test results will be shared. Most providers schedule follow-up visits to go over results. They only call with significantly abnormal results that cannot wait until the next follow-up appointment. Generally it is planned to review other results at follow-up office visits. If you ever have concerns about the result of a specific test, please call the office and request that the result be called back to you.

What happens if I am moving or changing providers and need my medical records?
Please complete our Medical Records Request Form and drop it off or mail it in to our building. If you prefer to handle this by phone, call our main number, 503-362-9334, or stop by the main receptionist desk and someone will assist you. In some cases, there are charges for records. After we have reviewed your request for records form, we will contact you if there are charges for these records. Please allow at least 2 weeks for records to be mailed.

What if I need forms completed?
Policies on this vary depending on the type of form and your provider’s policies. Many of them require a charge or an office visit to complete. Please print out the Request for Forms and mail it in or drop it by the front desk with your forms and we will contact you after your provider has had a chance to review the form.

What happens if I change my address or insurance company?
If you are here for a visit, please let the receptionist know when you check in for your appointment. You can also call our Billing Department through our main number, 503-362-9334 during regular office hours.
What Insurance Companies Do You Accept?
We work with most, but not all, Insurance Companies. Please check with our Billing Department before making any insurance changes. This way you can make sure your provider accepts your insurance plan.
